Unable to save FORM data after call has been hang up

Dear all,
I've setup a Goautodial system which seems to be working fine (campaign creation, agent log in to agent web interface, reporting, ...). Unfortunately after receiving a call the data in the form are not store in the database. So anytime the system receives a call from the same caller, the agent has to fill the form again.
Could you guide me on how I can get the data store so that anytime a call is received from a caller who is not new to the system, the previous data filled on the form by the agent appears. system is as follow:
GOadmin 3.3-1406088000
Vicidial 2.9 RC1 (2.9.441a)
Asterisk 1.8.23
CentOS 5.10 base OS
I'm new to Goautodial and wonder if there should be a submit button to save the data.

Try check your DID setting -> In-Group Call Handle Method set to = CIDLOOKUP and In-Group Agent Search Method = LB

If you encountering same problem please follow Regie's reply:
On your DID setting -> In-Group Call Handle Method set to = CIDLOOKUP and In-Group Agent Search Method = LB
