Probelm with calls (Lost calls, mute, etc)

Hello, I've could pass al my headache and fix almost everithing, but i have one error that i need to solve urgently
wen the agent is INCALL some times the call is mute, the call is conected, the client is on the phone, but the agent can't listen the client and vice versa,
they have to logout and log in to call again.
I have installed goautodial V4 on VPS Centos 7, Scratch Method.
If someone can help me please.

on Outbound, actualy i'm not using inboud yet.
To be honest i don't know how to configure inbound calls
