Agent log out, dial out and dead call ui

On v4.0 I installed from ISO and make an AMI for AWS.
Everything seems to be working, however:

Calls show dead air though they are live.
Agent logs out of dialer, but UI says they are still logged in to the webrtc phone
I tried dialing out in the web rtc box with 91NPANXXXXXX which matches my dial pattern for the carrier and it wont dial.

Please provide us asterisk screenshot when dialing for us to review it.
