GOautodial Open Source Omni-channel Contact Center Suite

Hi,
I have googled and tried many solutions to upload lead file as checking the permissions of file as '777' and tried to upload in .csv and .xls format in the same vicidial folder but I am not able to upload my leads file. there is error as

1). After click on upload lead button it shows : =================================================
Processing CSV: Comma Separated Values file...
LIST ID FOR THIS FILE: XXX
COUNTRY CODE FOR THIS FILE: 1

and

2). PHONE NUMBER: ""

3). click on "ok to process" button it gives pop-up with text:

"<B>ERROR: The file does not have the required number of fields to process it.</B> "
