CUSTOM FIELDS PROBLEM

Hi,
I'm with a problem that if i create more than 2 custom fields, i'm unable to upload leads, giving always error below:

Error: Lead File Not Compatible with List. Incompatible Field Names. Check the File Headers

Could you help me?

Hi,

Please check your list and make sure you are using the following field headers. Put your custom field right after the Comments column. Attached is our default list template.
