Time of this server has stopped, shown here time the last reboot (in addition).

can you post below command outputs
dahdi_cfg -vvv
screen -list
date
make sure to set the timezone to your actual timezone..
does the php and db time is right time or the other one is right time...

did u check whether the db is crashed
run the mysql repair command or do backup of mysql
mysqldump -p asterisk > asteriskbackup.sql
