1. in dialplan in carrier
2. in campaign dialprefix
3. leads upload with in_list name

make sure the admin id should be in righ User Groups, and admin interface options set to 1

can you post your asterisk cli log while makeing a call.
make sure your dialplan is updated in extension-vicidial....
